Photograph taken within the last six months

Proof of permanent residence in the country where the application is submitted.
For Japanese: A certificate of residence (JUMINHYO)
For non-Japanese: Proof of permanent residence in Japan

Financial evidence: Statement of transactions (TORIHIKI MEISAISHO) issued within one month for the last three months with a minimum balance of 500,000 baht per month

6. Certificate of criminal record clearance from the country of nationality of the country where the applicant has a permanent residence

Thai soft power-related activities
e.g., Muay Thai, Thai culinary training, and medical treatment

5. Proof of confirmation to attend the activity or letter of appointment from hospital/ medical center

6. Certificate of criminal record clearance from the country of nationality of the country where the applicant has a permanent residence

Spouses and children under 20 years old of DTV visa holders

5. Personal details of a DTV visa holder
e.g. Passport biodata page and the DTV visa approval of the DTV visa holder

6. Proof of relationship to the DTV visa holder
e.g. Family registration (KOSEKITOHON), or Certificate of residence (JUMINHYO)

7. Certificate of criminal record clearance from the country of nationality of the country where the applicant has a permanent residence
